Note on PDF forms:
If are using a fairly current version of Adobe Acrobat Reader,
you can fill in the form on your computer and sign it. Acrobat is
free, for PC's from http://www.adobe.com/reader . There's probably
an equivalent MAC version. Here's specific instructions for the
applications.
- Save the application to your system somewhere. Open it in
Adobe Reader. On the right is a choice for "fill and sign".
- You can fill in all the information on the application
page. For Adult applications, you can also fill in the approval
for background check page (except the signature). Fill in the
page(s), starting with the boxes for your name, all the way down
the page. IF there is a right column, fill it in also.
- Once you are sure all the text parts are good. Save and exit.
- Make a copy of the file you just saved. I suggest calling it
application-with-signature.pdf
- Open the copy: application-with-signature.pdf
- Go to the place to sign and click in the box.
- Select "sign" in the toolbar across the top, and make a
signature you like. Save it in the correct box.
- Go to the place to sign on Page 5 - the signature block
on the bottom left corner. Make another signature you like, and
save it in the box. Add a date to this one, which may have to be
done using the signature tool.
- Save the file and then exit.
- You can now send an email, and attach the file.
